MEDICAL INSURANCE AND HEALTH SAVINGS ACCOUNT (HSA)

NuVasive offers Shareowners three consumer-directed health plan options (sometimes referred to as High Deductible Health Plans or HDHPs) with differing deductibles and plan designs. NuVasive shares the premium cost for the Shareowner and eligible dependents. With these plans, NuVasive establishes a Health Savings Account (HSA) on behalf of the Shareowner to help offset the higher deductible. In addition, Shareowners contribute directly to an HSA are eligible for a Company matching contribution.

Beginning in 2017, Aetna will be the carrier for medical/RX plans other than for Shareowners in Ohio where the medical/RX carrier is Medical Mutual of Ohio (MMOH).

DENTAL INSURANCE

NuVasive offers Shareowners two Dental PPO coverage options through Delta Dental.

Standard PPO Plan: NuVasive shares the premium cost for the Shareowner and eligible dependents.

Enhanced PPO Plan: NuVasive shares the premium cost for the Shareowner and eligible dependents. This option provides a higher annual benefit and an orthodontia benefit for children up to age 19.

VISION INSURANCE

NuVasive offers Shareowners the ability to enroll in a 100 percent voluntary Vision PPO plan through VSP. The plan covers an eye exam, lenses, and frames every 12 months.

FLEXIBLE SPENDING ACCOUNT (FSA)

NuVasive offers Shareowners the opportunity to reduce their taxable income by participating in a FSA. NuVasive offers a Limited Purpose Healthcare FSA for Shareowners enrolled in a HDHP with HSA as well as a General Purpose Healthcare FSA for Shareowners not enrolled in a HDHP with HSA. We also offer Dependent Care and Transit FSA plans to all Shareowners.

401(k) PLAN

NuVasive offers a 401(k) plan through Voya. All Shareowners are automatically enrolled at 3 percent. Shareowners have the opportunity to contribute to a standard pre-tax account or post-tax Roth account. NuVasive offers a company match of 50% of the first 6% deferred (a 3% match), which vests over a three-year period.

EMPLOYEE STOCK PURCHASE PLAN (ESPP)

The NuVasive ESPP provides Shareowners the opportunity to purchase common stock (twice per year) at a 15 percent discount through convenient payroll deductions.

ON-SITE FITNESS CENTER

Many NuVasive facilities offer an on-site gym equipped with cardio equipment, weight machines, and free weights. Fitness classes led by certified fitness instructors are held at different times each day for Shareowners who would like to participate.

VACATION AND SICK LEAVE

Regular, full-time non-exempt (hourly) NuVasive Shareowners accrue vacation each pay period at a rate based on years of service.  Regular, full-time, non-exempt (hourly)  NuVasive Shareowners will receive 48 hours of sick leave at the start of each calendar year. Regular, full-time exempt (salaried) NuVasive Shareowners are eligible for our Flexible Time Off (FTO) program which provides for paid time off for vacation, personal time and sick leave.

HOLIDAYS

Regular, full-time NuVasive Shareowners will receive up to 12 paid holidays per calendar year in a combination of observed company holidays and floating holidays for use at the Shareowner’s discretion.  The actual observed company holidays vary across locations based on business needs.

Important Program Details:

  • Intern candidates must be currently enrolled at an accredited college or university pursuing a Bachelor’s, Master’s, or Ph.D. program
  • Successful completion of 2 years of undergraduate studies
  • Minimum GPA of 3.0
  • Candidates are responsible for their own housing and transportation during the internship
  • Must be eligible to work in the United States.
  • Summer intern positions are normally full-time (40/hrs) week for 10 to 12 weeks.
  • Off-Cycle intern positions are normally part-time (20/hrs week) for 3 to 6 months
