NuVasive Clinical Services provides the ability to schedule a case through the online form below (preferred), email or phone.
All same day and next day cases should be scheduled by phone at 800.638.7564.
Note: We request the patient’s demographics and insurance information at time of scheduling to facilitate compliance with federal and state surprise billing laws, including providing mandated disclosures to patients. Having this information also means NCS has the best opportunity to assign a participating monitoring physician to the case. If these fields are left blank, we will contact you to obtain this information
